How do I choose the correct event location?
When determining where an event occurred or where an unsafe condition existed, the event location is the place where the adverse event originated, not where the event was discovered. For example, a pressure injury that was discovered in the intensive care unit may have originated from the patient’s positioning in the operating room. With certain adverse events, such as infections, it may be difficult to determine precisely where the event originated. In these cases, select the location that your investigation has revealed as the most likely place.
How specific do I need to be in identifying the location?
When completing a Patient Safety Reporting Program (PSRP) report, select the answer that describes the location of the event as specifically as possible. For locations that fall under two categories, choose the more specific location. If a patient falls in a bathroom, select bathroom regardless of whether the bathroom was located at a preoperative area, the patient or resident’s room, the emergency room, etc.
