The Event Details section in the Patient Safety Reporting Program (PSRP) online reporting form collects and categorizes information about the patient prior to the event, and about the event itself. Under “Additional Event Information,” the “relevant surgeries and procedures” field determines which surgery or procedure was underway when the event took place (or was planned, if the event was an unsafe condition or near miss).
