List procedures connected or related to the reported event (i.e., those contributing to the event and those performed as a result of, or to prevent an event from occurring).

Additional Support

Which surgeries or procedures do I include under “Relevant surgeries or procedures”?

The Event Details section in the PSRP reporting form collects and categorizes information about the patient prior to the event, and about the event itself. Under “Additional Event Information,” the “relevant surgeries and procedures” field determines which surgery or procedure was underway when the event took place (or was planned, if the event was an unsafe condition or near miss).

Your response should only include the surgery or procedure that preceded the adverse event or that was in progress when the adverse event actually occurred. Your response should not include surgeries or procedures that had to be performed as a result of an adverse event.

For example, a patient was in the process of receiving an MRI, fell off the table, fractured their wrist, and underwent surgery to repair the broken wrist. In this case, the MRI is the relevant surgery or procedure. The wrist surgery to repair the fracture should be included in the “Complete Account,” located in the Event Summary section.
