Guide for Use

Enter the American Society of Anesthesiologists (ASA) Physical Classification System class assignment as documented in the medical record prior to the procedure. The class assignment is frequently found in a designated space on the anesthesia record.

If no class was assigned, select choice “Unknown.” Do not attempt to apply the ASA's class descriptions retrospectively.

Only events involving an invasive procedure for which ASA Class was not determined should mark “n/a.”
