Additional Support

Which surgeries or procedures do I include under “Relevant surgeries or procedures”?

The Event Details section in the PSRP online reporting form collects and categorizes information about the patient prior to the event, and about the event itself. Under “Additional Event Information,” the “relevant surgeries and procedures” field determines which surgery or procedure was underway when the event took place (or was planned, if the event was an unsafe condition or near miss).

For example, a patient was in the process of receiving an MRI, fell off the table, fractured their wrist, and underwent surgery to repair the broken wrist. In this case, the MRI is the relevant surgery or procedure. The wrist surgery to repair the fracture should be included in the “Complete Account,” located in the Event Summary section.
