Labels

  • Use form_field and any relevant segment labels (asc, hospital, nursingfacility, pharmacy)

  • Use the relevant section label (e.g., surgical_event, event_information)

    • Note: event-specific fields should be tagged with the associated event type (e.g., surgical_event, fall_event) and NOT with overarching header “event_information”)

  • Use required if the field is required to submit for all reports.

    • For example, “it is required if it appears” should not be labeled “required.”

  • Use essential if the field is a top priority for transferring the data dictionary.

    • Note: if the field is required, it will already be considered “essential” and doesn’t need a second label.

    • All “essential” labels will need to be removed before the data dictionary goes live.

Field Type

Guide for Use

List procedures connected or related to the reported event (i.e., those contributing to the event and those performed as a result of, or to prevent an event from occurring).

Additional Support

Which surgeries or procedures do I include under “Relevant surgeries or procedures”?

The Event tab Details section in the PSRP online reporting form collects and categorizes information about the patient prior to the event, and about the event itself. Under “Additional Event Information,” the “relevant surgeries and procedures” category field determines which surgery or procedure was underway when the event took place (or was planned, if the event was an unsafe condition or near miss).

...

For example, a patient was in the process of receiving an MRI, fell off the table, fractured his their wrist, and underwent surgery to repair the broken wrist. In this case, the MRI is the relevant surgery or procedure. The wrist surgery to repair the fracture should be included in the “Complete Account” sectionAccount, which is located under in the Summary tab.To provide more clarity, the prompt text in the reporting form for the “relevant surgeries and procedures” category has been updated. The text that appears below the question now reads: “Please list surgeries or procedures that preceded the reported event, or during which the reported event occurred (e.g., knee surgery, central line insertion).”Event Summary section.
