Responses

Hispanic

Select “Hispanic” if the patient was of Cuban, Mexican, Puerto Rican, South or Central American, or other Spanish culture.

Not Hispanic

Select “Not Hispanic” if the patient was not of Cuban, Mexican, Puerto Rican, South or Central American, or other Spanish culture.

Unknown

Select “Unknown” if the patient's Hispanic or Latino ethnicity was unknown.
